Supporting people with long-term conditions (LTCs) to take appropriate decisions is an integral component of care.

Professor Pinnock here shares how to complete and personalise action plans for individuals living with asthma or COPD.

Hilary Pinnock was a co-author of the 2014 meta-review of supported self-management in 14 LTCs, including asthma and chronic obstructive pulmonary disease (COPD). From this work came a taxonomy (Practical Reviews in Self-Management Support, or PRISMS, 2016) that provides a picklist (not a checklist) of strategies appropriate for personalising action plans.
